4/29/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C arrived to the home of Ben-Nita. Upon arrival to the home, Ben-Nita appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHP-C inquired about her day. Ben-Nita informed QMHP-C that her day was going well while inquiring about her behaviors since the last session. Mother reports she continues to struggle with controlling her anger. 2. Behavior: Ben-Nita participated in the session presenting normal and cooperative behaviors as she provided full participation with needing any redirection from QMHP-C. 3. Behavior: Ben-Nita participated in the session without incident. Ben-Nita struggled with drawing her thoughts; however, she was able to discuss with QMHP-C on how she will utilize coping skills to assist her with the stressors she is experiencing daily. Ben-Nita needed redirection from QMHP-C due to her getting angry with her mother when discussing her behaviors at the time of the session. 1.Intervention: QMHP-C began session by exploring Ben-Nita's emotional well-being. QMHP-C engaged Ben-Nita in a handout on anger control and how to best manage her verbal and physical aggressive behaviors. QMHP-C role-played scenarios with Ben-Nita for her to understand and practice positive anger management skills. QMHP-C then processed answers and scenarios with Ben-Nita. QMHP-C helped Ben-Nita identify various stimuli that has triggered her anger. QMHP-C assessed Ben-Nita in identifying thoughts, feelings, and actions that has characterized her anger responses. 2.Intervention: Ben-Nita was asked to list ways in which anger has negatively impacted her daily activities and interactions with others. QMHP-C engaged Ben-Nita in problem-solving, respectful behaviors and conflict resolutions. QMHP-C practiced above skills with Ben-Nita. QMHP-C assisted Ben-Nita with calming skills when feeling triggered and/or upset. 3. Intervention: Intervention: QMHP-C engaged Ben-Nita in a therapeutic art activity, encouraging Ben-Nita to draw her moods throughout the day and the different moods that she experiences. QMHP-C ended session by encouraging Ben-Nita to continue utilizing her positive coping skills and to utilize them when she recognizes a trigger and not once, she has begun displaying the anger. 1.Response: Ben-Nita appeared to be receptive to today's session. Ben-Nita struggles with understanding how her behaviors are projected toward others. The mother reported that Ben-Nita's behaviors have contributed to her anxiety and stress. 2.Response: Ben-Nita's behavior daily has been yelling, screaming throughout the house, talking back, being verbally disrespectful. Ben-Nita refuses to listen to directives and respect her mother limitations in the home 3. Response: During the therapeutic activity, Ben-Nita actively participated and identified emotions such as happiness, boredom, anger and frustration. At the end of the session, Ben-Nita stated that she was going to continue to utilize her positive coping techniques and that she enjoys utilizing the techniques even during times when she is not feeling angry.
